We are an organization offering group home independent living services for adults with mental health issues.

Recovery Management Services


Our Recovery Managers (RM) coordinate, link, advocate, monitor, and make referrals for each of our clients. We understand the best ways to leverage appropriate community resources to get our individuals what they need, when they need it. Using the Person Centered Recovery Plan, we work closely with each individual to learn their specific needs and provide them with a means to help them achieve their personal goals. Recovery management includes services to assist our clients in gaining access to needed Medicaid services; as well as medical, social, educational, and other resources regardless of funding sources. RMs are responsible for monitoring the provision of services included in the Individual Recovery Plan (IRP) to ensure that the clients needs, preferences, health, and welfare are promoted.

Comprehensive Provider


HCBS-AMH services are provided in “home and community-based settings” including homes, apartments, host home companion family residence, and assisted living facilities. Small community-based residence HCBS-AMH settings must meet certain qualifications.

Eligibility Requirements


HCBS-AMH focuses on three targeted populations. Applicants have to meet only one of the following targeted population.

Long term Hospitalization: A total of 1095 days in an in-patient psychiatric hospital during the last 5 years.
• Jail Diversion: 4 arrest and 2 psychiatric crises during the 3 years before the referral.
• Emergency Room Diversion: 15 emergency room visits during the 3 years before the referral.
To Be Eligible, Applicants Must: • Have a primary diagnosis of a serious mental illness (SMI)
• Be 18 years or older
• Be able to live in the community with HCBS-AMH services
• Meet clinical and functional criteria
• Medicaid Eligible
